Registration Form
This form can be used for all levels and divisions of baseball and softball at Sun Praire. Please use a new form for each child you are registering. Discount rates apply for 2 or more children.

Volunteer Application
All persons interested in volunteering for the Sun Prairie Little League must complete this form. This form is required by the National Little League of America.

Coach Application
COACHES NEEDED! The Sun Prairie Little League is seeking energetic, positive, and responsible individuals for coaching positions in our league. If you are interested in becoming a baseball coach or a softball coach please complete this application form. This application must be completed each year, even if you have coached with us previously. Completing this application does not guarantee a coaching position. IMPORTANT: The official Little League Volunteer Application Form must be completed in addition to this form (see above).

Umpire Application
The Sun Prairie Little League is seeking energetic, responsible, and disciplined individuals for umpire positions in our league. If you are interested in becoming an umpire please complete this application form. Adults and kids older than the age of 13 (thirteen) are encouraged to apply. Prior to the season, a brief training session is required.

Monetary Donations
Use this form for making a tax-deductable donation to the Sun Prairie Little League. Reminder, many employers have a matching donation program. Please contact your benefits coordinator for more information.
